Report of Pastoral Change in Congregation

An effort is made to report to the semiannual CMC business meeting changes in pastoral assignments which occur in CMC congregations. The purpose of this report form is to receive information needed for an adequate and accurate report and to place the information on file. Reference to pastoral assignments includes the assignment and function of senior pastor, associate pastor, ordained deacon, and preacher. This form is not intended for use in reporting changes in assignment of overseer or other functions which are not a part of pastoral leadership and preaching in the local congregation.

This refers to the pastoral arrangement before the change being reported here. Give the name and previous function (senior pastor, associate pastor, ordained deacon, or preacher) of each member of the pastoral team, including those whose function has not changed with the rearrangement being reported here.

List the name of each member of the pastoral team in the new arrangement and the function/assignment of each.
